DEPENDENT NOT ADDED TO SUBSCRIBER’S PLAN

Received denial from United Health Care stating dependent not eligible. Cld ---------  @ -------- and spoke with -----------------  said patient is not added to member’s plan and suggested we have the member call insurance and have the patient added to the plan. Last letter was sent to member on ------------ .

Please advice if we can confirm with member as to whether patient was added to policy or not.
